Proactive Adult Day-Care Program

Proactive offers both medical and social model adult day-care programs. The medical model is called Adult day health-care (ADHC) program, this provides not only the social activities associated with most senior programs but also the medical/nursing and rehabilitation services ordered by a physician that will help our registrants improve mobility and remain living in their own homes.

The social model offers elderly individuals and those interested in their general wellbeing a structured, active, friendly, and supervised environment where they can flourish.

Health and recreation services are provided in a caring and creative environment, designed for the care, comfort, and convenience of the elderly and younger disabled adults.

Proactive medical model program operates seven days a week, from 9:30 am until 13:30 pm. Flex hours are available. It is designed for elderly and younger adults who:

Have cognitive, physical, or psychological conditions requiring supervision including Alzheimer’s disease or other related dementias, coronary artery disease, diabetes, Parkinson’s disease, stroke, osteoarthritis, Multiple Sclerosis, Muscular Dystrophy, head trauma, etc.

What is adult day care?

Proactive Adult day care is a planned program of activities in a professional care setting designed for older adults who require supervised care during the day, or those who are isolated or lonely and desires companionship, those who cannot be safely left alone at home and those who lives with someone who works outside the home or who is frequently away from home for other reasons.

Proactive Adult day care program enables seniors to socialize and enjoy planned activities in a group setting, while still receiving needed health services.

At the same time, Proactive offer family caregivers respite from caregiving duties while knowing that their loved one is in a safe place.

The program provides older adults an opportunity to get out of the house and receive both mental and social stimulation, as well as the continuing care they need.

It gives caregivers a much-needed break in which to work, attend to personal needs, or simply to rest and relax.

Proactive Adult Day care program Social Model

Our social model program operates Monday through Friday from 10 am until 4 pm and is designed for the elderly and those interested to improve their overall wellbeing. The program is run by our compassionate and caring staff.

Our services are as follows:

  • Two meals and snacks
  • A variety of stimulating activities such as art exploration, computer skills, crafts, exercise, games, dancing, gardening, trips, holiday, and birthday celebrations
  • Assistance with activities of daily living
  • Transportation is available
  • A beauty salon/barbershop and a gift shop is provided.

Services for caregivers.

Proactive provide counselling, support groups for caregivers, help for care planning, and caregiving education.

We offer therapy and programs that include children, where older adults are encouraged to visit local classrooms and share their life experiences with the children, educating and enlivening both groups in the process.
